Okay. It was early PST and no caffeine on board when I first perused the site. My bad for missing the pun.

Curious about the mechanism of injury? The nail looks a bit large for a pneumatic gun.

It was a pneumatic nailgun. The great toe was nailed tight to the shoe, with the nailhead flush against the skin (actually indenting it) and the nail traversing the proximal phalanx. The guy had cut the top portion of the tennis shoe away with a boxcutter, but the sole wasn't going anywhere easily. It was a really bad hangnail, you might say. :]


My partner had already arranged for Ortho to take it out when I came on shift, but I had to go look of course. It wasn't my patient (and he no habla) so I didn't get a picture of the wound.

Been there. Done that. Lucky for me I had on steel toe boots so the nailed missed me and hit the guy next to me in the shin.

I saw that and thought it was the x-ray of my daughter's toe when she was 9. She was clearing a "construction pile" for my husband in her berkenstocks...stepped on a piece of wood and the nail went right through the shoe and through her big toe. We had to cut the piece of wood down to get her into the car and to the ER. The attending asked her if she wanted it numbed or to just "pull it out." We all opted for numbing...as we turned our heads to discuss the attending yanked the piece of wood off along with the nail off and out of her foot. I'd like to think they were just really busy and didn't want to take time to numb the toe...daughter cried and screamed a LOT...and to this day she doesn't like the ER...
